Each prospective Introductory member requires 5 sponsors. Please use this form to submit your sponsorship agreement for a prospective member. In order to be a sponsor, you must be an active regular or senior member. You may ONLY sponsor 1 prospective member in any calendar year.

Introductory Membership is subject to the availability of memberships and Board acceptance of the applicant.

A fully completed Membership packet must be submitted to the Membership Committee. No partially completed Membership applications will be evaluated to accepted.

If an application is rejected for any reason, sponsorship rights are returned to the Member.
