To request reimbursement for expenses from TLYCC, please use this TLYCC Reimbursement Form.
On a PC or Mac, click on the form to open it OR download the file and then open it with Adobe Acrobat Reader. You can then fill out the form and save it. Once you have completed the form, please email it and your receipts to: tlycc@allseasonsaccounting.
On an iPhone or iPad, tap on the pdf, and then you can ‘forward’ it to Adobe Acrobat Reader, Import it, fill it out and then ‘share’ so you can email it.
